School Nurse

ALL medication, prescription and over the counter, MUST be brought to the clinic by a parent/guardian/adult. ABSOLUTELY NO MEDICATION CAN BE BROUGHT IN BY A STUDENT!
In order for students to receive over the counter medication that is provided by the school, a blue medical form MUST be filled out and signed by the parent. Parents will not be able to give consent over the phone or through email. These forms will go home with back-to-school paperwork.
No over the counter medication will be given after 3:00 pm.
7th grade parents - please check your child's immunization records. Two immunizations are required for entrance to the 7th grade: meningitis and Td (tetanus) booster. Your child may have already received them, but please check with your physician. You can also call me and I can tell you if they are up to date.
